The incidence has been increased of 3.26 to 5.44/100.000 cases, from 1973 to the date. The multimodal therapy of nonseminomatous testicular cancer in clinical stage 1 (CS1) includes monitoring, nerve preserving retroperitoneal lymphadenectomy (PRL) and chemotherapy. The PRL has demonstrated to be curative in 90% of patients, still with presence of retroperitoneal metastasis of low volume. Objectives: To know retroperitoneal metastasis frequency and complications in patients with nonseminomatous testicular cancer clinical stage 1 subjected to PRL. Material and methods: From January 1995 to December 2005, in the Urology Service of the Hospital de Oncología, of the CMN Siglo XXI clinical files of patients with diagnosis of nonseminomatous testicular cancer, in clinical stage 1, put under PRL, were reviewed, analyzing demographic variables, presence of retroperitoneal metastasis and complications of the procedure. Results: 126 files of patients with diagnosis of nonseminomatous testicular tumor clinical stage 1 subjected to PRL were reviewed, registering: average of age was 24 years (17-44 years), and folloup of 54 months (7-127 months) we found right testicular tumor in 67 patients (53.2%); 36% were teratomas, 30.2% embrionary cancer, 28.6% teratocarcinoma, 3.2% viteline sac tumor and 1.6% cariocarcinoma. The histological study of retroperitoneal limph nodes was positive in 20.63% of the cases and the overall rate of complications was 19.9%. In 66 patients it was possible to evaluate the retrograde ejaculation appeared in 3 patients (5.5%). Conclusions: Patients with nonseminomatous testicular cancer CS1 must be put under a careful protocol of evaluation to determine the more appropriate treatment which can be monitoring, chemotherapy or PRL. Considering that the opportunity of cure is essential in these patients, the PRL is a reasonable treatment alternative for germinal cells tumors in clinical stage 1. Introducción: El cáncer testicular representa 1% de todos los tumores malignos, siendo el más común entre los 15 y 34 años de edad. La incidencia se ha incrementado de 3.26 a 5.44/100.000 casos, desde 1973 a la fecha. El manejo multimodal del carcinoma testicular no seminomatoso en etapa clínica 1 (EC1) incluye vigilancia, linfadenectomía retroperitoneal neuropreservadora (LRP) y quimioterapia. La LRP ha demostrado ser curativa en 90% de los pacientes, aun con presencia de metástasis retroperitoneales de bajo volumen. Objetivos: Conocer la frecuencia de metástasis retroperitoneales y complicaciones en pacientes con cáncer testicular no seminomatosos etapa clínica 1 sometidos a LRP. Material y métodos: En el periodo comprendido de enero de 1995 a diciembre 2005, en el Servicio de Urología del Hospital de Oncología del CMN Siglo XXI se revisaron los expedientes clínicos de pacientes con diagnóstico de cáncer testicular, no seminomatoso en etapa clínica 1, sometidos a LRP, analizándose variables demográficas, la presencia de metástasis retroperitoneales y complicaciones del procedimiento. Resultados: Se revisaron 126 expedientes de pacientes con diagnóstico de tumor testicular no seminomatoso etapa clínica 1 sometidos a LRP registrando: la mediana de edad fue 24 años (17-44 años), y la mediana de seguimiento de 54 meses (7-127 meses) encontramos tumor testicular derecho en 67 pacientes (53.2%); 36% fueron teratomas, 30.2% carcinoma embrionario, 28.6% teratocarcinoma, 3.2% tumor de saco vitelino y 1.6% coriocarcinoma. El estudio histológico de los ganglios retroperitoneales fue positivo en 20.63% de los casos y la tasa global de complicaciones fue de 19.9%. En 66 pacientes fue posible evaluar la eyaculación retrógrada que se presentó en tres pacientes (5.5%). Conclusiones. Los pacientes con cáncer testicular no seminomatoso en EC1 deben ser sometidos a un cuidadoso protocolo de evaluación para determinar el tratamiento más apropiado que puede ser vigilancia, quimioterapia o LRP.
