| Sumario: | A model of primary health care delivery that deals with the specific health concerns of a neighborhood is described. The model creates a partnership with the community and develops collaborative relationships with health care providers. Targeted health services are made available in a community-based, nurse-managed site. Clients are given the basic health services they require, local providers broaden their client base with “satellite locations,” and the principal medical institutions receive more suitable referrals and have less unnecessary use of the emergency rooms. This proposal tied for second place in the 1994 Secretary's Award for Innovations in Health Promotion and Disease Prevention competition.
