| Sumario: | Part of a special issue on aging and information technology. In order to tackle the problems of over- and undermedication and nonadherence to drug interventions and to offer patients the tools to better manage their medications, James Heffernan, medical director of HealthCare Associates, and his colleagues have joined forces with a group of organizations committed to creating a system in which patients and their health-care providers are connected to each other in a seamless communications network. This Senior-Med project aims to offer patients tailored information, social support, and links to their physician and pharmacist; to allow providers to constantly update and make immediately available to clinicians patient medication history and current prescriptions; and to allow for e-mail between patients and physicians. Details of the problems related to managing medications routinely prescribed, the project's specific aims and underlying assumptions, and the collaborators' expectations are provided.
