Sumario:Introduction: the opening access policy in Intensive Care Unit (ICU) is recommended by international evidences and supported by the Regional Health System of Emilia-Romagna. The aim of this research was to assess health-care quality provided in the ICU of AUSL Forlì, before and after the starting of the open access policy to the Unit, in order to evaluate the impact of this change on the perception of patients. Material and methods: a sample of 113 ICU patients were face-to face interviewed, using a 50-item questionnaire. 68 people were interviewed before the opening and 45 after. Results: the analysis of results showed that the main discomfort factors are different in the two groups. The first group reported pain, discomfort caused by the endotracheal tube and inability to move. The second group reported noise, disturbed sleep, disturb by medical activities and fear. The survey highlighted a common relief factor for both groups which was the presence of health-care workers. The presence of family members, receiving information, perception of closeness and emotional support were particularly relevant for the second group. In the second group wealso found higher rates of perceived quality of nursing-related topics, such as kindness, ability to reassure, willingness to give explanations and to listen to patients requests. On the other hand, a perceived negative element was the attitude of nurses to speak in front of patients as if they were not there. We also found out that patients appreciated the increasing of visiting time, which did not affect privacy issue. Conclusions: overall, the patients were satisfied with the service received in the ICU. The second group was much more "enthusiastic" than the other. The open access policy of the ICU could really have positive psychological and physical impacts, which are appreciated by patients.
Introduzione: una politica di visita aperta nei reparti di Terapia Intensiva è consigliata non solo dalla letteratura internazionale, ma anche favorita delle politiche delle Regione Emilia-Romagna. Obiettivo della ricerca è stato quello di raccogliere la valutazione sulla qualità dell’assistenza nella TI dell’Ausl di Forlì, sia prima sia dopo la decisione di attuare una politica di visita aperta. Materiali e metodi: sono stati intervistati, con intervista faccia a faccia (questionario di 50 item), 113 pazienti ricoverati nella TI, 68 quando la TI aveva una politica di visita chiusa (1 ora al giorno, obbligo di indossare camice, calzari, mascherina) e 45 dopo l’apertura del reparto (entrata libera nel pomeriggio e obbligo solo di lavarsi le mani). Risultati: rispetto ai fattori di disagio durante il ricovero, nel primo gruppo prevalgono il dolore, il tubo endotracheale e l’impossibilità a muoversi; nel secondo il rumore, il sonno disturbato, le attività mediche e la paura. In entrambi i gruppi emerge come principale elemento di sollievo la presenza costante degli operatori; nel secondo emerge però anche la capacità rassicurante della presenza dei familiari, delle informazioni ricevute, della vicinanza e del supporto emotivo. Nel secondo gruppo si nota inoltre un aumento medio della qualità percepita rispetto a tutti gli elementi relativi agli infermieri (gentilezza, disponibilità all’ascolto, capacità di tranquillizzare e dare chiarimenti). Cresce tuttavia anche la loro tendenza a parlare davanti al paziente come se lui/lei non ci fossero. Si può notare infine un apprezzamento da parte dei pazienti circa l’aumento del tempo dedicato alla visita dei parenti, che non incide negativamente sul rispetto delle privacy. Conclusioni: nel complesso i pazienti sono molto soddisfatti del servizio ricevuto in TI. I ricoverati del secondo gruppo sono nettamente più “entusiasti” degli altri. L’apertura della TI appare quindi un elemento non solo gradito dai ricoverati, ma in grado di favorire il loro benessere psico-fisico.
