Design: This was a retrospective study conducted at the Pharmacy Department, Hospital Universiti Sains Malaysia (HUSM), Malaysia. Materials and Methods: The prescription claims data from January to December 2009 were included in this study. Prescribing error was defined as any wrong or dubious information related to the drugs prescribed. Results: A total of 212 patients were prescribed with oxycodone and 44.3% were female. The mean age (SD) of the patients was 50.9 (15.30). A total of 286, 126 prescriptions were screened at the outpatient pharmacy and from that amount 517 (0.2%) were prescriptions that contained oxycodone. Four (1.9%) patients below the age of 18 were prescribed with oxycodone. One hundred and thirty two (25.6%) prescriptions had a wrong dosing frequency. Potential drug interaction was found in one hundred and fifty nine (30.8%) prescriptions while duplicate prescribing was identified in 19.3% of the patients. Conclusion: A number of prescribing errors were found in this study. These prescribing errors may result in increased number of unwanted events and healthcare expenses.
