The dissection of free air may not be confined solely to the mediastinum. The mediastinum communicates with the submandibular space, the retropharyngeal space, and vascular sheaths within the neck. In rare cases after dental procedures, subcutaneous emphysema and pneumomediastinum have been reported. In this article we report a case of a patient with iatrogenic pneumomediastinum after the dental extraction. Pnömomediastinum, mediastinumdaki serbest havanın veya gazın alveolar alandan ya da iletici hava yollarından köken alarak mediastinal alana geçmesi olarak tanımlanır. Mediasten içindeki serbest hava sadece alveolar alandan ya da iletici hava yollarından köken alarak oluşmayabilir. Submandibular boşluk, retrofaringeal boşluk ve bu alanlardaki vasküler kılıflar serbest hava ile ilişkilidir. Dental prosedürler sonrasında da nadir olarak subkutanöz amfizem ve pnömomediastinum görülebildiği rapor edilmiştir.
