This disease is a rare disorder and mainly affects healthy young males and also known as spontaneous mediastinal emphysema. It is generally known as a benign prognosis because of spontaneous resolution. Spontaneous pneumomediastinum typically presents with the sudden onset chest pain, dyspnea, subcutaneous emphysema, dysphagia, dysphonia and sometimes neck pain. Two cases who were diagnosed with SPM and had no predisposing factors are presented as a reminder in differential diagnosis in cases with sudden onset of shortness of breath and chest pain. Spontan pnömomediasten (SPM), travma, cerrahi veya diğer medikal işlemler olmaksızın mediasten içinde serbest hava bulunmasıdır. Spontan mediastinal amfizem olarak da adlandırılan bu hastalık nadir olup, özellikle genç sağlıklı erkeklerde ortaya çıkmaktadır. Spontan rezolüsyon olması nedeniyle genellikle benign bir süreç olarak bilinmektedir. Klinik tablo ani başlayan göğüs ağrısı, nefes darlığı, cilt altı amfizem, disfaji, disfoni ve bazen boyun ağrısı şeklinde ortaya çıkar.
