        Objective: We evaluated our results of extended cervical mediastinoscopy for the purpose of staging in patients who were previously diagnosed as left upper lobe or left centrally located non-small cell lung carcinoma (NSCLC) in our study. Material and Methods: Between 2005 and 2009, fifty seven patients who were considered to be resectable by means of preoperative radiological and laboratory investigations with the diagnosis of left upper lobe or left centrally placed NSCLC were included in the study. All patients underwent cervical mediastinoscopy and concurrent extended mediastinoscopy for staging. Results: Fifty seven patients underwent extended cervical mediastinoscopy, [53 male, 4 female, mean age 57,6 years (range 33-76 years)]. Sampling of lymph node number 5 was achieved in thirteen (22.5%) of 57 patients, sampling of lymph node number 6 was done in six (10.5%) and sampling of both lymph nodes 5 and 6 were done in 38 (67%) patients during extended mediastinoscopy. Overall 17 patients had positive mediastinal lymph node involvement. N2 disease was determined by extended mediastinoscopy in seven (12.2%) of these 17 patients. Standard cervical mediastinoscopy (SCM) and ECM showed multiple N2 disease in six (10.5%) patients and N2 disease was diagnosed in four patients by SCM. Sensitivity, spesificity, accuracy, positive predictive value and negative predictive value of extended mediastinoscopy was 86.6%, 100%, 96.4%, 100% and 95.4% respectively. No operative mortality was detected. Only one patient (1.7%) had partial pneumothorax and he was treated with tube thoracostomy. Conclusion: We conclude that to perform a complete mediastinal invasive staging in patients with left upper lobe or left centrally placed NSCLCs, extended mediastinoscopy with concurrent cervical mediastinoscopy is effective with the advantages of using the same incision with short operation time, high sensitivity, accuracy and negative predictive value and is safe and a useful technique with low morbidity and mortality rates.
        Amaç: Çalışmamızda, sol üst lob veya sol santral yerleşimli küçük hücreli dışı akciğer kanseri (KHDAK) tanısı almış hastalarda, evreleme amacı ile nadir uygulanan ekstended servikal me diastinoskopi sonuçlarımızı değerlendirdik. Gereç ve Yöntemler: 2005 ile 2009 tarihleri arasında, rezektabl sol üst lob veya santral yerleşimli KHDAK tanısı konulmuş 57 hasta çalışmaya alındı. Tüm hastalara evreleme amacı ile standart servikal mediastinoskopi (SSM) ve eş zamanlı olarak eksten ded servikal mediastinoskopi (ESM) yapıldı. Bulgular: ESM uygulanan 57 hastanın, 53’ü erkek, 4’ü kadın ve yaş ortalaması 57,6 (33-76) yıl idi. Hastaların 13 (%22,5)’ünde 5 no’lu lenf nodu, 6 (%10,5)’sında 6 no’lu lenf nodu, 38 (%67)’inde ise 5 ve 6 no’lu lenf nodları örneklendi. On yedi hastada pozitif sonuç elde edildi. Bu hastaların 7 (%12,2)’sinde ESM ile pozitif sonuç elde edilerek N2 saptandı. Altı (%10,5) hastada ise SSM+ESM ile pozitif sonuç elde edilerek multipl N2 tespit edilirken, toplam dört hastada SSM ile N2 saptandı. ESM'nin duyarlılığı %86,6, özgüllüğü %100, doğruluk oranı %96,4, pozitif tahmin değeri %100 ve negatif tahmin değeri %95,4 olarak bulundu. ESM uygulanan hastalarımızda mortalite izlenmedi, morbidite olarak bir (%1,7) olguda parsiyel pnömotoraks izlendi. Sonuç: ESM’nin KHDAK’li hastalarda ayrıntılı bir mediastinal evreleme için, SSM ile aynı insizyondan uygulanabilmesi ve bu nedenle ameliyat süresini kısaltması, yüksek sensitivite, doğruluk, negatif öngörü değerleri ve düşük morbidite oranlarıyla güvenli, yararlı bir teknik olduğunu düşünüyoruz.
