However, the incidence and mortality rate appear to be increasing, especially in high-risk populations such as human immunodeficiency virus (HIV)- positive men who have sex with men (MSM), and patients with organ transplantation-associated immunosuppression. High-risk types of human papillomavirus (hr-HPV) have been well established in the pathogenesis of many anal squamous cell carcinomas, intraepithelial precursor lesions, and their corresponding cytological abnormal results on anal smears. This review outlines the developing role of anal cytology, the Bethesda system for interpretation of anal cytology and how it differs from its cervicovaginal counterparts, and the controversies associated with anal cytology test performance, high-resolution anoscopy, and anal biopsy.
        Le carcinome du canal anal est une pathologie rare au Canada. Il faut savoir toutefois que les taux d'incidence de ce cancer et de mortalité liée à ce cancer sont en hausse selon toute évidence, particulièrement dans les groupes à risque élevé comme les hommes infectés par le virus de l'immunodéficience humaine (VIH) qui ont des relations sexuelles avec des hommes et les greffés en état d'immunosuppression. Il est établi maintenant que les papillomavirus à haut risque oncogène sont en cause dans la pathogenèse de nombreux carcinomes malpighiens du canal anal et de lésions intraépithéliales précancéreuses comme en témoignent les résultats anormaux de l'étude cytologique du frottis anal. L'article examine la place grandissante de la cytologie anale, l'utilité de la classification Bethesda dans l'interprétation des résultats de la cytologie, la différence entre la cytologie anale et la cytologie cervicovaginale et la controverse que soulèvent la performance des tests de cytologie anale, l'anuscopie de haute résolution et la biopsie anale.
