Summary:Recently updated guidelines indicate that molecular testing can be performed on circulating tumour DNA (ctDNA) to rule in EGFR mutation in patients with lung adenocarcinoma who are treatment-naïve or showed progression on first- or secondgeneration tyrosine kinase inhibitors. Pathologists may be asked to advise upon the suitability of ctDNA testing as an alternative to tissue-based testing and to discuss ctDNA test results in the context of tissue studies. An understanding of the rationale for ctDNA testing and its role in the molecular testing pathways is necessary for integration of this new technology into clinical practice.
Selon les mises à jour récentes de plusieurs lignes directrices, il est désormais possible de réaliser une analyse moléculaire de l'ADN tumoral circulant (ADNtc) afin de confirmer une mutation de l'EGFR chez les patients atteints d'un adénocarcinome des poumons n'ayant jamais reçu de traitement ou chez qui on a constaté une progression sous traitement par des inhibiteurs de la tyrosine-kinase de première ou de deuxième génération. Les pathologistes pourront être amenés à dispenser des conseils quant à la pertinence d'une analyse de l'ADNtc comme solution de rechange à une analyse tissulaire, et à débattre des résultats d'une telle analyse par rapport aux études tissulaires. Il est nécessaire de bien comprendre ce qui justifie une analyse de l'ADNtc et le rôle de cette dernière dans le déroulement des différentes analyses moléculaires, afin d'intégrer cette nouvelle technique dans la pratique clinique.
