        Pulmonary sequestration is a congenital lung malformation characterized by a segment of pulmonary tissue that does not communicate with the tracheobronchial tree or pulmonary arteries. When the separated segment is enveloped by its own pleura, it is referred to as an extralobar pulmonary sequestration (EPS). The vascular anatomy of EPS is variable, but typically drains into the systemic or pulmonary veins. We present an unusual case of EPS with venous drainage into the portal vein. Due to the atypical vascular anatomy, a virtual 3D display of the lesion was created to facilitate preoperative planning, a method not previously described in the workup of EPS. A printed 3D model was also used for patient teaching. The EPS was subsequently successfully resected via left thoracotomy.
        La séquestration pulmonaire est une malformation congénitale des poumons caractérisée par un segment de tissu pulmonaire dissocié de l'arbre trachéobronchique et des artères pulmonaires. Lorsque le segment est délimité par sa propre enveloppe pleurale, on parle de séquestration extralobaire (SEL). Sur le plan vasculaire, il existe des variantes anatomiques, mais la SEL se draine en temps normal dans la circulation générale ou les veines pulmonaires. Nous présentons ici un cas atypique de SEL avec drainage dans la veine porte. Étant donné cet élément anatomique inhabituel, une image virtuelle en 3D de la lésion a été créée afin d'appuyer la planification préopératoire, une méthode jamais décrite auparavant dans le bilan d'une SEL. Un modèle imprimé en 3D a également été utilisé pour informer la patiente. La SEL a ensuite été réséquée avec succès par thoracotomie gauche.
