Sumario:BACKGROUND: Martorell's hypertensive leg ulcer was first described in 1945 as an extremely painful ulcer lesion and erythematous-violaceous edges on the lower limbs, initially associated with uncontrolled systemic arterial hypertension, later related to type 2 diabetes and obesity, in women between 50 and 70 years of age. CLINICAL CASE: A 33-year-old female patient with a history of poorly controlled hypertension and grade II obesity, who initially presented a spontaneous blister-like lesion at the left plantar level, progressing to a very painful left pretibial necrotic ulceration, denying trauma at that level. The ankle brachial index of the affected limb was 1.2 mmHg. Doppler ultrasound did not show an obstructive process; an incisional biopsy identified a characteristic arteriolosclerosis, hyalinosis and atrophy at the subcutaneous cellular tissue level; integrating the diagnosis of hypertensive ulcer of Martorell. Surgical debridement was performed with significant pain relief, she was a candidate for the application of local therapy with negative pressure continuously, favoring the appearance of granulation tissue. CONCLUSIONS: In Mexico, due to the high prevalence of systemic arterial hypertension, type 2 diabetes, and obesity, it is suggested to consider Martorell's hypertensive ulcer a possible cause when treating ulcerative lesions in this group of patients, with the aim of not delaying diagnosis and treatment.
ANTECEDENTES: La úlcera hipertensiva de Martorell fue descrita en 1945 como una lesión ulcerosa extremadamente dolorosa y bordes eritematovioláceos en los miembros inferiores, asociada inicialmente con hipertensión arterial sistémica descontrolada, posteriormente con diabetes tipo 2 y obesidad, en mujeres entre 50 y 70 años. CASO CLÍNICO: Paciente femenina de 33 años con antecedente de hipertensión arterial sistémica de mal control y obesidad grado II que manifestó una lesión tipo ampolla de manera inicial a nivel plantar izquierdo, que progresó a una ulceración necrótica a nivel pretibial izquierdo, muy dolorosa; negó traumatismos en esa zona. El índice tobillo-brazo de la extremidad afectada fue de 1.2 mmHg, el ultrasonido Doppler no demostró proceso obstructivo. La biopsia incisional identificó arterioloesclerosis, hialinosis y atrofia en el tejido celular subcutáneo, con lo que se estableció el diagnóstico de úlcera hipertensiva de Martorell; se realizó desbridamiento quirúrgico con alivio importante del dolor; la paciente era apta para la aplicación de terapia local con presión negativa de manera continua, lo que favoreció la aparición de tejido de granulación. CONCLUSIONES: En México, debido a la alta prevalencia de hipertensión arterial sistémica, diabetes tipo 2 y obesidad, se sugiere considerar a la úlcera hipertensiva de Martorell una posible causa al tratar lesiones ulcerosas a fin de no retrasar el diag- nóstico y el tratamiento.
