Twenty-five months into this new job, he was fired for "professional incompetence and lack of integrity." Shortly afterwards, the College of Physicians and Surgeons of Nova Scotia reprimanded him, precluding future medical practice in Canada. A year and a half after his dismissal in Canada his earlier practice, at Alder Hey, came under intense scrutiny, with the initial focus on thousands of post-mortem paediatric organs he had illegally accumulated. A parliamentary inquiry was established to investigate the full scope of his Liverpool practice and eventually resulted in a 535-page inquiry report. The Alder Hey organ hoarding scandal led to increased regulation of human tissue use in the UK, including the Human Tissue Act of 2004 and the establishment of the Human Tissue Authority regulatory body in 2005. This historical study compares and contrasts his practice in both countries. The Royal Liverpool Children's Inquiry had been initially directed at investigating allegations that van Velzen ignored autopsy consent limitations and practised illegal organ hoarding. The inquiry definitively confirmed these facts. In contrast, van Velzen rigorously abided by autopsy consent limitations and did not illegally hoard organs in Halifax, where his questionable activities had been identified as dishonesty, data fabrication, and corner-cutting. While the inquiry determined that he was guilty of these same behaviours in Liverpool, these were only discovered as a by-product of investigating his autopsy infractions. In contrast, these behaviours were the root causes of his dismissal in Halifax. Although 25 years have passed since his dismissal in Halifax, no publication has documented van Velzen's brief practice in Canada. What happened, and how was he dealt with so quickly?
