| Sumario: | This article focuses on the global rise of type 2 diabetes and the challenges in translating prevention and management evidence into effective, sustainable practice. It highlights the Novo Nordisk Network for Healthy Populations (NHP) at the University of Toronto, established in 2021, which employs implementation science to address diabetes through a multi-level framework targeting healthcare delivery, community engagement, and structural determinants of health. Using Peel Region in Ontario, Canada, as a living lab, the NHP collaborates with diverse stakeholders to co-develop culturally sensitive interventions and policy changes aimed at reducing health inequities. The article emphasizes the need for integrated, population-based strategies that address social determinants and environmental factors alongside individual behaviors to combat the diabetes epidemic.
